Log into your account your username your password Forgot your password? That seems to do the trick, although if there was an easier/quicker way that would be fantastic. But no worries! This error is because you do not have access to the published location of thetemplate or the template has not been published.

I recently implemented a company wide system of online forms, through a sharepoint server.
